No, I don't think it's just A4e.
I don't know how much malpractice auditors will find, though. If the government wants to hear about malpractice, it should ask for whistle blowers to come forward. They could be ex-employees or ex-clients. I think one reason for the focus on A4e is basically that there are especially many disgruntled ex-clients.
A former employer of mine actively bullied it's staff into fraudulent activities. All totally off the record and against policy, but it was the top management that were personally doing the bullying.
